O., PhD, Associate Professor, Department of Paediatrics Introduction:Angiotensin-converting enzyme (ACE), or kininase II, plays an important role in the development of systemic lupus erythematous (SLE) because its end product, angiotensin II, plays an integral role in the regulatory system responsible for endothelial control and vascular tone by activating renin- angiotensin- aldosterone system (RAAS). This is a membrane-bound enzyme, which is a zinc-dependent peptidyl dipeptidase localized in the vascular endothelium, mainly in the lungs, as well as in the tissues of the kidneys and, to a lesser extent, in the tissues of other organs. Its first role is to split off the C-terminal dipeptide, whereby it converts angiotensin I to angiotensin II (A-II). The second is in the destruction of the hypotensive peptide - bradykinin. Further, ACE2 can cleave A-II to angiotensin (1-7). Both substances (A-II and A(1-7)) are bioactive peptides that affect different systems. A-II acts through the angiotensin type I (AT1) receptor, mediating functions such as vasoconstriction, increased insulin resistance, oxidative stress (OS), chronic inflammation, hypertension, and target organ failure. Thus, an imbalance in the work of the RAAS can initiate or exacerbate the cardiac and nephrotic symptoms of systemic lupus erythematous. The aim of the study: to determine the level of ACE in the blood of patients with SLE. Materials and methods: 16 patients aged 13.88 ± 0.60 years with SLE were examined. The duration of the disease was 50.50 ± 11.49 months. All patients received basic therapy, including azathioprine, hydroxychloroquine, and glucocorticoid, for more than 12 months. The control group consisted of 7 healthy adolescents aged 14.40 ± 0.81 years. To determine the level of ACE in the blood, the FAPGG kinetics turbidimetric method with the Angiotensin converting enzyme test system was used. Normal values of the ACE level were considered values in the range of 13.3 - 63.9 U/l. Statistical data processing was done using SPSS17 applications (license 4а180844250981ae3dae-s/nSPSS17) for IBM PC Pentium-4. Results: in patients with SLE, the ACE level was significantly lower than in healthy adolescents and amounted to 22.22 ± 3.69 U/l, p < 0.01 versus 40.51 ± 6.55 U/l, while being within the limits of the normal values of this indicator. From previous studies it is known that the level of ACE in the blood of patients with sarcoidosis is reduced under the influence of corticosteroid therapy. Conclusion:Thus, in adolescents with systemic lupus erythematous, a normal level of angiotensin-converting enzyme was established, but this may be due to the use of glucocorticoids therapy. 15 АКТУАЛЬНІ ПИТАННЯ СУЧАСНОЇ МЕДИЦИНИ ANTIBIOTIC RESISTANCE CRISIS Bharat Makkar V. The rapid emergence of resistant bacteria is occurring worldwide, endangering the efficacy of antibiotics, which have transformed medicine and saved millions of lives. Many decades after the first patients were treated with antibiotics, bacterial infections have again become a threat. The antibiotic resistance crisis has been attributed to the overuse and misuse of these medications, as well as a lack of new drug development by the pharmaceutical industry due to reduced economic incentives and challenging regulatory requirements. The Centers for Disease Control and Prevention (CDC) has classified a number of bacteria as presenting urgent, serious, and concerning threats, many of which are already responsible for placing a substantial clinical and financial burden on the U.S. health care system, patients, and their families. Coordinated efforts to implement new policies, renew research efforts, and pursue steps to manage the crisis are greatly needed. The management of microbial infections in ancient Egypt, Greece, and China is well-documented. The modern era of antibiotics started with the discovery of penicillin by Sir Alexander Fleming in 1928. Since then, antibiotics have transformed modern medicine and saved millions of lives. Antibiotics were first prescribed to treat serious infections in the 1940s. Penicillin was successful in controlling bacterial infections among World War II soldiers. Antibiotics have not only saved patients’ lives, they have played a pivotal role in achieving major advances in medicine and surgery. They have successfully prevented or treated infections that can occur in patients who are receiving chemotherapy treatments; who have chronic diseases such as diabetes, end-stage renal disease, or rheumatoid arthritis; or who have had complex surgeries such as organ transplants, joint replacements, or cardiac surgery. The aim of the study: The aim of our work was to study the issue of antibiotic resistance, which in our time is a very urgent problem. Materials and methods. We analyzed the work of scientists on antibiotic resistance, which has been carried out for many years. We have studied the publications of scientists around the world. We studied publications from such authoritative sources as Centers for Disease Control and Prevention (CDC), Harvard Health Publishing Harvard medical school, Mayo Clinic, USA. Results. As early as 1945, Sir Alexander Fleming raised the alarm regarding antibiotic overuse when he warned that the “public will demand [the drug and] … then will begin an era … of abuses.” The overuse of antibiotics clearly drives the evolution of resistance. Epidemiological studies have demonstrated a direct relationship between antibiotic consumption and the emergence and dissemination of resistant bacteria strains. Antibiotic-resistant infections are already widespread in the U.S. and across the globe. A 2011 national survey of infectious-disease specialists, conducted by the IDSA Emerging Infections Network, found that more than 60% of participants had seen a pan-resistant, untreatable bacterial infection within the prior year. Many public health organizations have described the rapid emergence of resistant bacteria as a “crisis” or “nightmare scenario” that could have “catastrophic consequences.” Methicillin- Resistant Staphylococcus Aureus was first identified five decades ago. Since then, MRSA infections have spread worldwide, appearing at a high incidence in several countries in Europe, the Americas, and the Asia-Pacific region.MRSA infections can be very serious and are among the most frequently occurring of all antibiotic- resistant threats. In the U.S., 11,285 deaths per year have been attributed to MRSA alone. Vancomycin-Resistant Enterococci presents a major therapeutic challenge.Enterococci cause a wide range of illnesses, mostly among patients in hospitals or other health care settings, including bloodstream, surgical-site, and urinary tract infections. VRE infections, often caused by Enterococcus faecium and less frequently by Enterococcus faecalis, have a lower prevalence and epidemiological impact than MRSA does worldwide, except for the U.S. and some European countries. Drug-Resistant Streptococcus pneumoniae. S. pneumoniae can cause serious and sometimes life- threatening infections.It is a major cause of bacterial pneumonia and meningitis, as well as bloodstream, ear, and sinus infections. Drug-Resistant Mycobacterium Tuberculosis infections are a serious threat in the U.S., and an even more urgent threat worldwide. The WHO reported that in 2012, 170,000 people died from drug-resistant tuberculosis infections. Carbapenem-Resistant Enterobacteriaceae are a group of bacteria that have become resistant to “all or nearly all” available antibiotics, including carbapenems, which are typically reserved as the “treatment of last resort” against drug-resistant pathogens. Conclusion. Despite the fact that information about the features of the course of COVID-19 has been studied in sufficient detail, the criteria for predicting (CP) the severity of the course of the disease have not been developed sufficiently. The purpose of the work. Identify CP the severity of the course of COVID-19 by developing a mathematical model. Materials and methods. 60 patients aged 18 to 85 who were inpatient treatment at the RCIDH were examined, the group of patients 18-50 consisted of 40 without comorbidities, with a mild and moderate course of COVID-19. Patients in the age group of 51-85 had a moderate and severe course. The results. In patients aged 18-50 an acceleration of ESR by 48%, an increase in the level of prothrombin index (PI) by 14%, and D-dimer by 31% were found before treatment (BT). In the group 51-85, leukocytosis was detected in 57%, lymphocytosis BT in 9%, after treatment (AT) in 92%, the level of PI BT was within the normal range in all patients, with a decrease of 83% AT, BT neutrophilia and increased D-dimer in 73% and 84% of patients, AT reduction in neutrophils and D- dimer in 93% and 57%, BT ESR was greater than normal in 100% BT and 87% AT. The level of leukocytes (LL), ESR and D-dimer had a linear dependence and a statistically significant effect on the course of COVID-19. In patients aged 51–85, there were direct correlations between comorbidities and a complicated course: the level of lymphocytes, monocytes, ESR, D-dimer. The infrastructure of correlations of indicators BT and AT and comorbidities revealed direct correlations between Diabetes Mellitus (DM) and the level of plateles (LP) BT (r=0.7); DM and the level of monocytes (r=0.95), D-dimer (r=0.7) BT; Coronary heart disease (CHD) and the level of lymphocytes BT (r=0.97); obesity and D-dimer BT (r=0.83); fever with the LL (r=0.85), nausea and the LP (BT r=0.92, AT r=0.98); D-dimer BT (r=0.98). There were inverse correlations between DM and LL BT (r=-0.97); СHD and LL BT (r=- 0.85); fever and the level of lymphocytes BT (r=-0.97). Construction of regression equations based on trend lines revealed that AT patients had a decrease in the level of D-dimer (by 20%), an increase in the LP (by 30%), a decrease in the LL (by 26%), neutrophils (by 38%), an increase in the level of lymphocytes by 3 times, a decrease in the level of ESR exceeded the norm by 1.5 times. CP were calculated for each prognostic function based on cluster and regression analysis. A linear dependence and a statistically significant effect on the course of COVID-19 of the LL, ESR, and D-dimer were established. In the group aged 51-85, there was a strong direct relationship between comorbidity and the complicated course, namely: LL and lymphocytes (p<0.001), PI (p<0.01), ESR (p<0.001), D-dimer (p<0.001). GROIN HERNIA: ANATOMICAL RISK FACTORS FOR THE RECURRENCE Bugaj J. The analysis of scientific publications of last 5 years shows that the questions of anatomical preconditions, role of gender and physique in recurrences of groin hernias after mesh hernia repair remain up-to-date. Considered as a “gold standard”, tension-free hernioplasty has many advantages comparing to tissue repair, but the question of the graft’s size remains a subject of discussions. The aim of the study. To substantiate anatomical parameters influencing choice of graft`s size and shape in groin hernia repair. Materials and methods. In the research took part 74 patients of both gender with primary groin hernias. In all patients were measured the following parameters: the distance from jugular notch to pubic symphysis, the distance from xiphoid process to pubic symphysis, the distance between the lower edges of the costal arches, and the inter ASIS distance (between anterior superior iliac spines). For all the patients the vertical and horizontal indexes of the body were determined. All the patients were divided into 3 groups due to type of physique on the basis of obtained data: 26 patients (35.1%) of endomorph type, 20 patients (27%) of ectomorph type and 28 patients (37.8%) of mesomorph type. An in-depth analysis of anatomical parameters of the groin region was performed in these groups. The spinal distance, the height of hypogastrium, the distance from the pubic symphysis to anterior superior iliac spine, the distance from pubic tubercle to the deep inguinal ring were determined. Received data were used to calculate the optimal sizes of the prosthetic mesh for the groin hernia repair depending on the physique and gender. Results. The obtained data show that the anatomical sizes of pelvis and groin regions in particular vary not only in people of different gender and body structure, but sometimes may vary in one person. That leads us to necessity of individual approach to the choice of size of hernia mesh in every individual case. The results of the measurements of anatomical parameters were used to calculate the optimal size of the allograft for the groin hernia repair for the patients of different physique and gender. We may conclude that groin allografts of standard size (6×11 cm) correspond not to all patients. During the operation, surgeons have to modify the size with the help of scissors or to use a bigger piece of mesh to form the graft of necessary size. All that may lead to mistakes and raise the risk of complications and recurrence. 19 АКТУАЛЬНІ ПИТАННЯ СУЧАСНОЇ МЕДИЦИНИ Conclusions. The aim of the study. To establish the features of endothelial function of the heart in adolescents-boys with AH, who have no signs of metabolic risk. Materials and methods: The study included 22 boys with hypertension (AH) aged 13–17 years without obesity and overweight. The mean age was 15.2±1.3 years. The comparison group included 19 healthy young men of the same age. The study included an assessment of clinical examination, anthropometry, ECG, heart ultrasound, and the level of total cholesterol in blood serum. The function of the vascular endothelium was determined by a test of reactive hyperemia on the brachial artery using Doppler echocardiography. Results. In boys with AH at rest, systolic blood pressure (SBP) averaged 156±4.5 mm. rt. Art., diastolic (DBP) - 90.0 ± 4.4 mm. rt. Art. In the comparison group, BP was within normal limits, SBP averaged -105.9 ± 3.5 mm. rt. Art., DBP -62.9±1.6 mm. rt. Art. The indicator of total cholesterol in children with AH was within the normal range and amounted to 4.5±0.4 µmol/l, which did not differ from the level in the children of the comparison group. In the test with reactive hyperemia, the increase in the diameter of the artery in children with AH averaged 7.6% [7.6; 12.0], and was significantly less than those in the healthy group - 15.62% [6.38; 20. 5] (p<0.05). In 58.4% of young men with AH, insufficient endothelium-dependent vasodilation was found. Adolescents with endothelial dysfunction were taller, which averaged 175.4 ± 2.3 cm (versus 173.7 ± 0.7 cm, F=0.054, р˂0.05). BMI in group 1 adolescents was 20.8±2.25 kg/m2 versus 20.0±3.6 kg/m2 in comparison group. It drew attention to the fact that 40% of adolescents in group 1 had long limbs. An increase in the thickness of the interventricular septum (0.8±0.3 cm versus 0.7±0.1 cm, p0.05), mass of the left ventricular (LV) myocardium (119.9±16.0 g versus 106.3±6.4 g; p˂0.05), increased pressure in the left atrium (0.89 ± 0.17 mm Hg versus 0.81 ± 0.05 mm Hg; p ˂ 0.05) and higher ejection fraction of the left ventricular (73.1±1.8% versus 66.2±1.3%; р0.05) in children with AH and XІХ Міжнародна наукова конференція студентів, молодих вчених та фахівців 20 endothelial dysfunction compared to children with normal endothelium-dependent vasodilation. Conclusion. Therefore, children with AH often have endothelial dysfunction, in which the increase in the diameter of the artery is not only less than normal, but also half as much as in healthy children. This is the result of an imbalance of endothelial factors and other mechanisms that are activated at puberty, causing an increase in blood pressure. The deterioration of endothelial function is accompanied by increase in the LV ejection fraction. MORPHOLOGICAL AND SOCIODEMOGRAPHIC TENDENCIES OF NON- NEOPLASTIC HEART MUSCLE DISEASES IN ENDOMYOCARDIAL BIOPSIES (EMBS) Delon I.N. Prof. Dr., Head of Department Pathological Anatomy. Introduction: EMBs are used since the late 60s to investigate the viability of the heart muscle. EMBs continues to be the gold-standard of investigation for heart muscle pathologies. Still, in the last 2 decades advances in technological investigation and tools rose a question about the efficacy of EMBs. This thesis aims to investigate the tendencies of EMBs with the main group of indications throughout the years 2017-2021, in Kaunas Clinics, considering the different pathologies, the amount of sample material, gender, age, technique of investigation. Furthermore, comparison before and after Covid 19 year was made. Aim:To characterize morphological and socio-demographic tendencies of non- neoplastic diseases cardiomyocytes and interstitial tissue in endomyocardial biopsies, comparing these tendencies before and after the break of Covid-19 infection. Materials and methods: EMB’s of 223 cases with non-neoplastic myocardial pathology were selected and statistical analysis using SPSS software was applied. EMBs were classified according to categories of main indication, patient’s general data, amount of material taken, findings, which technique was reported and year of procedure. As a setting, year 2020 was chosen as Covid-19 infection break year, and tendencies of collected data were compared with other years by applying the same categories. Statistical significance p<0.05. Results: As it was hypothesized, the most common indication for EMBs was „allograft rejection reaction surveillance”, which matches the worldwide accepted tendencies. Diving deeper, it was detected that less material was taken from female patients than male patients with the same indication above the age of 65 (p<0.05). However, the most unexpected finding was the correlation between the Covid-19 outbreak and the rise of performed EMBs amongst men in the age group above 65 years old (p<0.05), this finding supporting the hypothesis that Covid-19 infection can be a factor providing favorable conditions for minor chronic diseases to become 21 АКТУАЛЬНІ ПИТАННЯ СУЧАСНОЇ МЕДИЦИНИ more pronounced and become more severe, requiring more aggressive health care approach, especially among the elderly male population. As for the yearly trends, there were no significant shifts between the years 2017-2021 about the use of EMB as a diagnostic tool (p>0.05). Conclusion: This study confirmed that the most common indication for EMB is in the field of heart transplant field diagnostics, where it serves, so far, as irreplaceable tool for monitoring the structural functionality of transplanted heart muscle. Tendencies of increasing EMB rates for diagnostics of heart muscle pathology in elderly patients around 2020 demonstrate an overall impact of Covid-19 infection to healthcare services, also presenting a possible non-direct side effect of Covid-19 to aggravation of clinical course in underlying chronic diseases among elderly patients. CARDIOVASCULAR REMODELING IN PATIENTS WITH ARTERIAL HYPERTENSION COMBINED WITH TYPE 2 DIABETES MELLITUS Ezekwu P.K. Essential arterial hypertension (AH) remains one of the urgent problems of modern cardiology, despite significant advances in its prevention and treatment. AH quite often occurs in combination with type 2 diabetes mellitus (DM), which significantly increases the risk of cardiovascular complications. The aim of study. The aim of the investigation was to study the features of left ventricular (LV) myocardial remodeling and the thickness of the intima-media of the common carotid artery (TCA) in patients with hypertension combined with type 2 diabetes. Materials and methods. Studied outpatient cards of patients 84 patients (39 women and 45 men) with AH stage II and 2nd degree. The average age of the patients was 49.4 ± 4.5 years. In accordance with the developed study design, the patients were divided into 2 groups: group 1 (n = 41) patients with hypertension without type 2 diabetes; Group 2 (n = 42) with a combined course of hypertension and type 2 diabetes. The control group consisted of 20 healthy people matched by sex and age to the examined patients. Clinical examination of patients included the determination of anthropometric parameters, fasting glycemic parameters (GKN), total cholesterol (TC), and low-density lipoprotein cholesterol (LDL-C), high- density lipoprotein cholesterol (HDL-C), triglycerides (TG). The analysis of the results of echocardiography was performed using the diagnostic system "Mindray DS-N3", a transducer with a frequency of 2.0-5.0 MHz according to standard techniques according to the recommendations of the American Society of Echocardiography (2015). The data of ultrasound examination of common carotid arteries with measurement of the thickness of the complex (TIM CCA) were analyzed. Left ventricular myocardial hypertrophy (LVH) was detected in 98% of patients in group 2 and in 93.55% of patients in group 1 (p <0.05). In patients with AH and type 2 diabetes, concentric LVH was dominant (68.54%), while in patients with AH, concentric LV remodeling (47.42%). When assessing the TIM of CCA, the mean values in patients with hypertension were 0.85 ± 0.05 mm, with a combined course of the disease 0.96 ± 0.07 mm (p <0.05). In patients with hypertension and type 2 diabetes, direct correlations of the TIM indicators of CCA with BMI (r = 0.44, p <0.001), systolic blood pressure (r = 0.48, p<0.001), total cholesterol (r = 0.48, p <0.001), LDL-C (r = 0.44, p <0.001), inverse correlations were established with HDL-C (r = 0.42, p <0.001). Conclusions. The study of outpatient records of patients with arterial hypertension and type 2 diabetes mellitus found that patients with comorbidity of these diseases observed the most unfavorable variants of myocardial remodeling, more pronounced thickening of the TIM of the CCA than in patients with arterial hypertension without diabetes mellitus, which necessitates timely correction of blood pressure and metabolic disorders. The question of rehabilitation of patients after a stroke, especially against the background of cardiac pathology, is very relevant at the present time. Scientists believe that the best time to start rehabilitation is the first hours and days after the incident. The correct position of the patient at the pre -hospital stage, turning over in bed and breathing exercises during hospitalization — all this is the beginning of rehabilitation. The justification for early rehabilitation is the data of modern neuroimaging ­methods , which showed that the processes of neuroplasticity , which lead to the reorganization of functions, are the basis of their recovery and have the most intensive course in the first days and weeks after a stroke. The tasks of rehabilitation after a stroke are the fight against hypokinesia and hypodynamia, the prevention of neurological complications of the acute period, such as an increase in the severity of spasticity in paretic limbs with the appearance of contractures, the appearance of speech disorders, motor postures and stereotypes. The aim of study. The purpose of the study is to study the state of cardiac activity in the post- stroke period. Materials and methods. The health status of patients who were undergoing rehabilitation after an ischemic stroke and had chronic heart pathology was studied. Among the main subtypes of ischemic stroke, some are related to heart pathology - cardioembolic and hemodynamic . Cardioembolic stroke was found in 22% of patients with ischemic stroke. At the same time, as a rule, insufficiency of collateral blood circulation is detected. During the rehabilitation measures, safe movement procedures were used: passive gymnastics, light active gymnastics, respiratory gymnastics, positional treatment, neuromuscular stimulation, light massage. It was found that cardiac pathology significantly affects the post- stroke stage both in the early and later periods. Decompensation of previously existing cardiac pathology, including anginal attacks, heart failure, heart rhythm disturbances, can worsen rehabilitation processes. A correlation was also found between the localization of brain damage and cardiac disorders. Heart rhythm disorders are also important, they have a negative impact on reparative processes in the area of cerebral ischemia. It was found that frequent supraventricular extrasystole causes a decrease in cerebral blood flow up to 7%, ventricular - up to 12%, and ventricular paroxysmal tachycardia - up to 40-75%. Conclusions. Therefore, under the condition of adequate therapeutic correction and somatic stability, the presence of chronic heart pathology should not hinder the scope and pace of rehabilitation measures. DIAGNOSTIC VALUE OF MYOCARDIAL ELECTROLYTES IN CASES OF SUDDEN CARDIAC DEATH Fomin D. Sudden cardiac death is the leading cause of death in all communities of the European Union but its precise incidence is still unknown. Due to poor morphological evidence a part of sudden cardiac deaths remains unexplained after forensic medical investigation and these cases are referred to a sudden unexplained death. The aim of the study. The aim of present study was to verify the usefulness of the determination of potassium and sodium in heart tissue defining sudden cardiac death. Material and Methods. The retrospective research was performed at the Vilnius branch of The State Forensic Medicine Service. The autopsy protocols for five years were analysed and the cases of sudden cardiac death were selected, a forensic autopsy and toxicological blood and urine test had been performed. Cases of the sudden death of non-cardiac origin, cases of cardiomyopathy of various origins, and cases of acute cardiac arrest of unspecified origin were excluded. During autopsies XІХ Міжнародна наукова конференція студентів, молодих вчених та фахівців 24 heart samples were taken from different sites of cardiac tissue of left ventricle: the apex and anterior, lateral and posterior walls, anterior and posterior papillar muscles and the middle of the interventricular wall. Autopsy samples for trace element analysis were digested in the presence of concentrated nitric acid and perhydrol. The standard method of flame atomic emission spectroscopy was used to study the concentration of potassium and sodium. The data collected was processed using R software. Differences with P values less than 0.05 were considered significant. Results: The study sample consisted of 226 cases (165 males and 56 females). The mean age of the subjects was 47,2 years (range 2-97 years). In the case of ischemic heart disease prominent peaks in electrolyte disturbances have been observed in ischemic areas. In the case of acute toxic effects of alcohol, meanwhile, minor electrolyte disturbances were expressed evenly throughout the myocardium. Conclusions. In the cases of sudden cardiac death the significant disturbance of electrolytes in heart tissue can be detected. Forasmuch study showed that sudden cardiac death can not be related in a high degree of coronary artery arteriosclerosis, we suggest that the main cause of cardiac death in the investigated group was arrythmias due to electrolytes abnormalities in cardiac tissue. The ionic K/Na ratio in heart tissue allows distinguishing the sudden cardiac death and death due to acute toxic effects of alcohol. The biochemical mechanism for various types of K and Na disbalances in heart tissue in sudden cardiac death cases is not entirely clear and requires further investigation. For some people, hepatitis B is mild and lasts a short time. These “acute” cases don’t always need treatment. But it can become chronic. If that happens, it can cause scarring of the organ, liver failure, and cancer, and it even can be life-threatening. The aim of the study: Hepatitis B treatment: new immunotherapy 'highly effective' against hepatitis B. Materials and methods: We have studied the publications of scientists around the world. We studied publications from such authoritative sources as Centers for Disease Control and Prevention (CDC), Harvard Health Publishing Harvard medical school, Mayo Clinic, USA. We aimed to identify a treatment target to directly inhibit the virus while also boosting the immune cells fighting it. 25 АКТУАЛЬНІ ПИТАННЯ СУЧАСНОЇ МЕДИЦИНИ Cholesterol is a lipid (fat) that we ingest every day in our diets and that can exert multiple functions within different cells of the body. HBV infects the liver, an organ highly enriched in cholesterol and well known for limiting local immune responses.In the some of study, using human liver disease tissue samples in vitro, Professor Maini's lab at UCL showed that ACAT inhibitors boosted human antiviral T cells capable of eliminating the virus. This response is in contrast to currently available therapies. The immune-boosting effect was especially striking in T cells found in the HBV-infected liver and within liver cancer, overcoming the local restraints on immune cell function, allowing the T cells to target both the virus and cancerous cells. The Maini group then collaborated with Professor Jane McKeating's lab at the University of Oxford to show that ACAT inhibitors could also block the HBV life cycle in a way that other antivirals are unable to. These drugs therefore have a unique combination of antiviral and immunotherapeutic effects. Commenting on the findings, first author Dr Nathalie Schmidt (UCL Division of Infection & Immunity), said: "We have found a highly effective novel target for the treatment of chronic hepatitis B virus infection and liver cancer. Current treatments for chronic HBV infection (CHB) include pegylated-interferon- α (Peg-IFNα) and nucleos(t)ide analogues (NAs), but neither are suitably efficient in providing functional cure,4 a virological and clinical situation defined by undetectable levels of hepatitis B surface antigen (HBsAg) and HBV DNA in the serum, normality of alanine aminotransferase (ALT) and development of hepatitis B surface antibody (anti-HBs). The term “functional cure” is thus used to define a virological situation in which, like in patients who resolved acute hepatitis B infection, HBV is not fully eliminated and few hepatocytes harbor covalently closed circular DNA that is maintained under a repressed translational control by innate and adaptive immune mechanisms. Peg-IFNα can achieve sustained off-treatment control, but only in a limited proportion of patients; approximately 10%. NAs therapy suppresses the production of new virions, reduces HBV DNA to undetectable levels in the serum and leads to a normalization of transaminases. However, loss of HBsAg is rarely achieved. Furthermore, since NAs suppress HBV replication by targeting at the level of DNA synthesis only, they are ineffective in their ability to eradicate the cccDNA, the episomal form of HBV from infected cells. Treatment with NAs is thus considered lifelong with the potential risk of long-term toxicity, with limited data on treatment withdrawal, which results in reactivation of HBV in the majority, likely due to a defect on the restoration of anti-HBV immunity. Results: The cholesterol-modifying drug is already known to be safe in humans and we hope that study of this problem informs the development of clinical trials combining cholesterol modulation with other immunotherapies. In summary, findings offer exciting new possibilities for the treatment of patients with chronic viral infections and cancer. The risk of inducing severe liver inflammation has so far restricted direct clinical testing. The new methods of T cell engineering and criteria of CHB patient selection for immunotherapy should be revised, and such options can open new and safer avenues to test these new immunotherapeutic approaches. HEALTH COMPETENCE IN STUDENTS OF ELEMENTARY SCHOOL Kavya Kapil V.N. Cross-curricular teaching of health issues and health-promotion activities in school enables students to build health competence that can be described as the personal set of training, skills, experience, and knowledge and the ability to apply them to effectively manage his/her health behaviors and health outcomes. The aim of the study. To assess the level of health competence in students of elementary school. Materials and Methods. The study included 40 2nd-grade students. The health competence was considered a complex formation included health literacy, health attitude, and orientation, and health-promotion activity that tested by the Questionnaire "What is health?" (G.A. Khakimova), picture test "Houses" (O.A. Orekhova, adapted by G.A. Khakimova), Dembo-Rubinstein self-esteem test, students’ interview and pedagogical observation of students’ attitude to health- promotion actions. Results. It is established the following distribution of students of elementary school to the level of health literacy: 5% – advanced, 25% – proficiency, 50% – basic, and 25% – below the basic level. It means the majority of students had enough differentiated knowledge about the health issues but defined only 1-2 of its components (mainly physical health); they unmistakably identified threats to human health in a certain situation; they were convinced of the necessity to follow a healthy lifestyle as a cause of future health, but only within its physical aspect. It was shown that students of elementary school were distributed by the level of health attitude and orientation in the following way: 25% – advanced, 20% – proficiency, 20% – basic, and 35% – below basic. Hence, health did not occupy a leading place in the values hierarchy in a third of students in elementary school. As a result, they had an indifferent and even often negative attitude towards the performance of preserving and strengthening health actions (exercise, hardening, etc.), which was manifested by a lack of interest, refusal to perform, and crying. They could not independently and objectively evaluate their own behavior and the behavior of other people. Analysis of health-promotion activities determined that 30% of students in elementary school had advanced, 25% – proficiency, 20% – basic, and 15% – below 27 АКТУАЛЬНІ ПИТАННЯ СУЧАСНОЇ МЕДИЦИНИ the basic level. Consequently, a third of students took an active part in various activities aimed at preserving and strengthening health (exercise, hardening, physical competence, etc.) with great pleasure and on their own initiative and because of awareness of the expediency and necessity of these health measures. To sum up, elementary school students were distributed according to the levels of health competence such as 20% of students had inherent unconscious competence, 25% – of conscious competence, 30% – of conscious incompetence, and 25% – of unconscious incompetence. Conclusion. Health education and health promotion activities in elementary school should be strengthened in the direction of providing health literacy and health attitude and orientation. Adrenal tumors (AT) are common. Modern image techniques occupies a central place in the diagnosis of these tumors. After diagnosis of an AT, a multidisciplinary team theoretically clarifies the diagnosis quickly and prescribes appropriate treatment for the patients. This is usually performed in tertiary hospital (university clinic in our case). The aim of the study. We attempted to analyse retrospectively, how this system works in real life. Materials and Methods. We analised medical documentation of 150 patients (53 women and 97 men), who had adrenal tumors diagnosed by computer tomography or/and by magnetic resonance imaging. Essential clinical and laboratory (hormone assessment) data were documented. Age,heigh, weight, BMI, systolic and diastolic blood pressure, pulse were recorded together with results of investigation of DHEAS, testosterone, TTH, aldosterone, ACTH, fT4, cortisol, renin, metanefrine, normethanefrine, aldosterone/renin ratio. Age of women and men at the moment of AT diagnosis was the same (60,9 and 60,5 years respectively). No difference of systolic and diastolic blood pressure, pulse and BMI was found between women and men. Results. Concentration of aldosterone (around 70%), ACTH (around 40%), cortisol (around 70 %), renin (around 60%) , methanephrine and normethanephrine (around 65%) and aldosterone/renin ratio (around 25%) were investigated with the same frequency both in women and men. TTH, fT4 and DHEAS were assessed in women more frequently in women, than in men (48, 41 and 50% and 32, 28 and 40% respectively). We seek to assess oncohematological patients‘ serological response to the BNT162b2 mRNA vaccination and compare the immunogenicity between different hematological malignancies and treatment regimens. Methods: We conducted a nationwide prospective cohort study to assess the serological response to one and two doses of the BNT162b2 (Comirnaty®, Pfizer- BioNTech) vaccination in healthy healthcare workers (HCW) and patients with hematological malignancies (HM). Eligible participants were adult individuals, who received two vaccine doses, and had available biobanked blood samples from before vaccination and after the second vaccine dose administration. Abbott Architect SARS-CoV-2 IgG Quant II chemiluminescent microparticle assay was used to quantify serum anti-SARS-CoV-2-S1 IgG antibody (anti-S1 IgG antibody) concentrations 0–10 days before the first BNT162b2 vaccine dose (timepoint 0), on the day of second immunization (around day 21, timepoint 1), and 7 to 21 days after the second immunization (timepoint 2). Results: A total of 885 patients with hematological malignancies were enrolled between January 8 and April 21, 2021. An aged-matched comparison of 67 HCWs and 315 patients with HMs demonstrated, that after receiving two doses of the BNT162b2 vaccine, patients with HMs presented significantly poorer median anti- S1 IgG antibody responses than the healthcare workers (median 6961 AU/mL [IQR 1292–20672] vs 21395 AU/mL [14831–33553]; p<0.0001). Patients actively treated with Bruton tyrosine kinase inhibitors (BTKIs; n=44; 0 AU/mL [0–7]; p<0.0001), ruxolitinib (n=16; 10 AU/mL [0–45]; p<0.0001), venetoclax (n=10; 4 AU/mL [0– 1218]; p=0.0005), or anti-CD20 antibody therapy (n=87; 17 AU/mL [1–2319]; p<0.0001) had significantly worse anti-S1 IgG antibody responses after two BNT162b2 doses than patients with hematological malignancies who were not receiving active treatment (n=53; median 5761 AU/mL [629–16141]). In contrast, patients treated with tyrosine kinase inhibitors (n=41; 10537 AU/mL [IQR 2335– 29 АКТУАЛЬНІ ПИТАННЯ СУЧАСНОЇ МЕДИЦИНИ 19388]) and patients after autologous or allogeneic hematopoietic stem cell transplantation (HSCT; n=192; 6203 AU/mL [1451–16834]) demonstrated the best antibody responses to vaccination. During a median follow-up of 94 days (IQR 73– 102), there were three COVID-19-related deaths and nine documented SARS-CoV- 2 infections among fully immunized HM patients. Conclusions: Hematological malignancy patients display diverse, in comparison with healthy individuals poorer antibody responses to immunization with the BNT162b2 mRNA vaccine. Patients actively treated with BTKIs, ruxolitinib, venetoclax, or anti-CD20 antibodies are the most negatively affected and are at high risk of severe SARS-CoV-2 infection. Strict adherence to non-pharmacological prophylactic measures and household member vaccination remain of high importance for these immunocompromised individuals, as indicated by observed breakthrough infections after full immunization. The study of changes in the structural components of the gastric mucosa under the influence of endogenous and exogenous factors remains a significant problem that attracts clinicians and scientists. The aim of the work. To determine the morphofunctional changes of the gastric mucosa of rats with simulated acute inflammation. Materials and methods. Acute experimental inflammation was modeled by the introduction of λ-carrageenan, which belongs to phlogogens and causes structural changes in the mucous membrane by its mechanism of action. Following all bioethical norms, biopsies were removed on the experiment's 1st, 2nd, 5th, 14th, 21st, and 30th days, with subsequent sealing in paraffin according to generally accepted methods. Semi-thin sections were made from paraffin blocks, which were studied using a light microscope with a digital photomicroscope Biorex 3 (serial number 5604). Results. On the 1st, it was established that epitheliocytes of the integumentary pit epithelium underwent dystrophic changes, manifested by vacuolization of the cytoplasm, and decreased in the number of secretory granules. On the 2nd day of our observation, dystrophic changes intensify. There is a progressive decrease in the number of secretory granules, and areas of "desolation" appear in the cytoplasm. Irregularly shaped nuclei containing decondensed chromatin were identified in the center of the cell. On the 5th day of the experiment, epitheliocytes are characterized by the absence of secretory granules. There is a local desquamation of surface-pit cells. On the 14th day, areas covered with a thick layer of mucus were found. Nuclei are located in the central part of the cell and contain mostly decondensed chromatin and one nucleolus. Basophilic cytoplasm of refined honeycomb appearance and non-homogeneous optical density. On the 21st day of the experiment, the integrity of the covering epithelium is restored. Cells form a continuous layer. Oval-shaped nuclei are found in the basal parts of the cytoplasm. Decondensed chromatin is in the center of the nuclei. A significant number of mitochondria and secretory granules are visualized in the apical cytoplasm. On the 30th day of the experiment, a complete recovery of the layer of surface pit epitheliocytes took place. Сonclusions. Thus, we established that the administration of λ-carrageenan to experimental animals caused the structural components of the mucous membrane of the stomach, which had a staged character, to be manifested by alteration (dystrophy and desquamation of surface pit epitheliocytes and glandular gland cells), exudation (cellular reaction in the lamina propria) and reparation and corresponded to the main pathomorphological manifestations of acute experimental inflammation with a term of implementation of the inflammatory process up to 30 days. Pathogenic variants in QRICH1 (Glutamine-rich protein 1) gene leading to dysmorphic features and cognitive disability were described for the first time in 2018 and named by discoverer as Ververi-Brady syndrome. According to recent studies glutamine-rich protein 1 is likely to be involved in apoptosis and inflammation as well as hypertrophic differentiation required for normal longitudinal bone growth. Common clinical characteristics include speech delay, intellectual disability, tendency to short stature and microcephaly as well as mild dysmorphic features. Case report. 17-years-old girl was referred to geneticist to exclude inherited leukodystrophies due to variable clinical symptoms and dubious findings on magnetic resonance imaging (MRI). She is a first and only child born to healthy non- consanguineous parents of Caucasian origin. Her mother gave no history of previous illnesses or substance misuse during pregnancy; however, she has history of multiple miscarriages. On the 14th day after birth proband has had proportionally short stature, multiple haemangiomas on face, neck, and extremities as well as small tongue and narrow and high-arched palates. Her further motor and cognitive development were slightly delayed. On the furtherer examination proband’s height was 155 cm, weight 36 kg and body mass index (BMI) was 14,98 kg/m2. Also, occipital balding was observed. On neurologic examination she had mild spastic paresis in her right leg with initial contractures in Achilles’s tendon. She attended elementary school, however had difficulties with concentration and focusing on tasks. Her mother has similar dysmorphic features and social interaction peculiarities although has not reported any complaints. Brain magnetic resonance imaging (MRI) was performed in proband, revealing periventricular, symmetric hyperintense white matter lesions in both hemispheres. Considering radiologic findings and clinical presentation, inherited leukodystrophy was suspected in both proband and her mother and whole exome sequencing was performed which identified novel heterozygous variant in QRICH1 gene NM_017730.3:c.337C>T; p. (Gln113*), classified as pathogenic according to the ACMG (American College of Medical Genetics) 2015 criteria. Conclusion. Considering novelty of the disorder, every new clinical case contributes to enlargement of VEBRAS (Ververy-Brady syndrome) cohort expanding phenotypical and mutational spectrum with potential improvement in further care and observation of probands and their offspring.
